Transitional Care is specialized medical care for people with advanced illnesses. Its focus is to provide patients with relief from the stress of an advanced illness regardless of the diagnosis. Palliative services are appropriate at any age of life and at any stage of an advanced illness.
Routine hospice home care includes intermittent visits from all the members of the Interdisciplinary Team. A patient will be placed at this level of care if they reside at home (or a long-term care facility) and the symptoms of their illness are controlled with medication and treatments.
Our caregivers can provide respite care, or temporary relief for caregivers. This allows time to recharge, go to work, run errands, or simply take a much-needed break to avoid burn-out or frustration.
Our round-the-clock level of care is provided in the home for brief periods of time when the patient is experiencing a crisis. It is designed to honor a patient’s wish to remain at home by providing the care needed to control the patient’s symptoms and alleviate the crisis.
